VillageCare is a pioneering and innovative continuing care organization that offers post-acute care, community services and managed care options to people living in New York City.  We strive to understand the current state of need of those we serve and pursue appropriate responses.

Recognizing and supporting self-directed, interactive care enables the people we serve to control aspects of their own care and help them to maintain their independence.

For over 40 years, we have continually reached out to hard-to-serve, vulnerable and marginalized populations who may not have had adequate access to quality health care and rehabilitative services.

Serving New York City for Over 40 Years

VillageCare began in 1977 as a project by community volunteers to rescue and reorganize a for-profit nursing home slated for closure.  It has become a much larger organization that provides post-acute care, community-based services and managed long-term care.  As a result of this history, VillageCare has become a valued resource for the people we serve, their caregivers and other provider organizations with which we partner.  

VillageCare staff have extensive knowledge and experience in offering and coordinating care services that are recognized for high quality, as well as excellent outcomes and exceptional customer satisfaction by, among others, the New York State Department of Health and the federal Centers for Medicare & Medicaid Services (CMS).  These programs include VillageCare Rehabilitation and Nursing Center and VillageCare at 46 and Ten Medicaid Assisted Living Program.

In addition, VillageCareMAX offers a Medicaid Managed Long-Term Care Plan (MLTC), as well as two plans for individuals that are dually eligible for Medicare and Medicaid.

“Redefining Wellness” refers to our commitment and constant focus on an innovative and steadfast response to the continuing care needs of New Yorkers. For over 40 years, VillageCare has remained at the forefront of New York City’s health and wellness initiatives. Serving thousands of individuals each year, we are motivated by their ever-changing needs. Through award-winning services, as well as innovative partnerships, we strive consistently to enhance and better the services that we provide. As the health care landscape in New York continues to evolve, we reaffirm two organizational commitments to remain a viable partner and provider for years to come:

Celebrating Our Past, Building Our Future

Our storied history begins with the Greenwich Village community joining together in the late 1970s to save the only nursing home serving the West Side and Lower Manhattan.  It proved to be the springboard for a bold venture in long-term care, as well as care for those with chronic illnesses.  

This enduring enterprise is embodied in the programs of today’s VillageCare, the parent organization that grew from the community’s endeavor four decades ago.

1977

Not-for-Profit Village Nursing Home established after proprietary nursing home closes. 

1985

Board initiates planning to respond to AIDS epidemic.

1988

20th Street AIDS Adult Day Health Care Program opens. It was the first of its kind and served as a model for programs nationwide. 

1991

Opened an HIV/AIDS-specific Certified Home Health Agency.

1995

Opened an HIV/AIDS-specific Community Case Management program. Rivington House, an AIDS-specific nursing facility opens. It included a second AIDS Adult Day Health Care Program. 

1995

Village Care of New York (VCNY) Inc. created as a parent organization. 

1996

Short-term rehabilitation unit opens at Village Nursing Home. 

1998

Village and Chelsea Adult Day Health Care Programs open. 

2001

An Assisted Living Facility opens at 46th Street and 10th Avenue. 

2006

A Diagnostic & Treatment Center opens at 20th Street location. 

2009

VCNY and all of its programs rebrand with the name VillageCare to help better align all services. 

2010

VCRN opens in late 2010. A state-of-the-art post-acute care facility.

2012

Case management transitioned into VillageCare's Health Home and APS programs. 

2012

VillageCareMAX launches and is the first approved MLTC in New York State's Medicaid Reform efforts. 

2014

Awarded a $9M CMS grant for Rango, a virtual treatment adherence app for individuals with HIV/AIDS.

2017

VillageCareMAX expands its Medicare product line to include both a DSNP and MAP. 

2019

VillageCareMAX opens its first-ever Member Information Center in the West Village

2019

VillageCareMAX opens its Flushing Community Center
